Lab Card®
The Lab Card Program gives you and your dependents the option of receiving 100%, covered outpatient laboratory testing if your doctor sends your lab work to LabOne for processing. Lab Card is an optional program. If you choose not to use Lab Card, you will not be penalized. You will simply pay the deductible, coinsurance or copay portion of your lab work.
Lab Card does not replace your current healthcare benefits, it simply gives you and your dependents the option of receiving 100% coverage for outpatient laboratory testing. Because the lab card offers laboratory testing at a significant savings over other labs, the health plan pays the full cost of the covered laboratory tests and still saves money.
Lab Card covers most outpatient laboratory testing included in your health insurance plan, provided the tests have been ordered by a physician and processed at Quest Diagnositcs .
Outpatient lab work includes:
- Blood testing (e.g., cholesterol, CBC)
- Urine testing (e.g., urinalysis)
- Cytology and pathology (e.g., pap smears, biopsies)
- Cultures (e.g., throat culture)
Lab Card does not cover:
• Lab work ordered during hospitalization
• Lab work needed on an emergency (STAT) basis and time-
sensitive, esoteric outpatient laboratory testing such as fertility
testing, bone marrow studies and spinal fluid tests
• Nonlaboratory work such as mammography, X-ray,
imaging and dental work
To receive 100% covered lab testing:
- Show your Lab Card/healthcare card to the receptionist and to the person collecting your specimens. Tell them that your testing must be done by Quest Diagnositcs for you to receive free laboratory work. Your physician’s office can call the Quest Diagnositcs 800 number on your card if they need more information
- The Lab Card Program does not apply to members that have Medicare B as their primary insurance
