The Compass Rose Benefits Group (CRBG) is extremely excited to welcome the Intelligence Community to its suite of insurance products and services! Effective
24 July, all Intelligence Community (IC) civilian employees will have an opportunity to enroll in the CRBG Supplemental Insurance Plans located on the left side of this page to include:
- UBLIC Term Life Insurance (see "Important Information" on this plan listed below).
- Group Accident Plan - Use the link on the left side of this page for information and the application form for Accident Insurance. You will also need to complete a debit form and mail or fax both the application and the debit forms to the address below.
- Income Replacement (see "Important Information" on this plan listed below). This plan has undergone some changes and is now called the Long Term Disability Plan.
- Long Term Care - Use the link on the left side of this page for information about Long Term Care.
Note: Your premiums will be collected via an electronic debit from your checking/saving account.
Additionally, effective this Health Open Season ( 13 November – 11 December), IC civilian employees will be able to enroll in the CRBG sponsored health plan health plan known as the Association Benefit Plan (ABP)! Features of the ABP include:
- Highly Competitive Premiums
- Number One in overall member satisfaction for all the National fee-for-service Plans for 2005.
- $10 copay for doctor's visits
- $5 Generic prescriptions
- Discounts on Dental, Vision, hearing, podiatry, chiropractic, and alternative medicine at
NO ADDITIONAL COST.
Important Information pertaining to Term Life Insurance Plan and Long Term Disability Insurance (formerly called Income Replacement Plan)
Term Life Insurance (UBLIC)
You can enroll in UBLIC Term Life Insurance for coverage amounts up to $100,000 without meeting medical underwriting when you enroll within 31 days of your hire date. If you are interested in amounts higher than $100,000, you will only have to meet medical underwriting for the amount of coverage in excess of the first $100,000. Use the link on the left bar to access the UBLIC Application Form. Also click on the debit form link and fax or mail it with the completed application.
Long Term Disability(LTD) (formerly called Income Replacement Plan)
CRBG is changing the name, benefits and features of this plan. Look under "Related Links" to view the features of the revised plan and to print an enrollment form. (Do not use the link on the left hand side of this page under Supplemental Plans).
Enrollment Instructions
- Look under "Related Links" to view plan features and print enrollment forms and bank debit authorization form.
- Completed forms to include the enrollment form and payment deduction information form can be sent to CRBG three ways:
- By using your IC "Internal" fax and sending it to 703-613-7807
- By using an external commercial fax at 703-734-7013
- Send by mail external mail (USPS) to the following address

- Upon receipt, CRBG will verify eligibility, process the application and contact you directly via email or telephone confirming the effective date of coverage. At time of confirmation you will be advised of the prorated amount that must submitted be to CRBG prior to the beginning of automatic deductions from your savings/checking account. Automatic deductions will begin in September.
